KeepMEWell is offered by the Maine Center for Disease Control and Prevention (Maine CDC).  No personal information about you, such as your name or phone number, will be collected that would allow anyone to identify you.  All information is kept anonymous (no one will know who you are) and confidential (your answers will not be shared). If you choose to email your report to yourself, your email address will not be saved. 

However, your answers will be grouped with others who use the KeepMEWell website and reviewed by Maine CDC staff to identify Maine public health needs and to plan and evaluate public health efforts aimed at meeting those needs.
